2008/2009 Annual Statement of Economic Interests Filing for the Period of January 1, 2008 to December 31, 2008
It’s That Time of Year – Welcome to Filing Season!
To assist you with your filing requirements, the City Clerk’s Office has developed this web page to provide you with all of the necessary resources in order to understand your obligation and reporting requirements. Additionally, unique forms based upon your individual designations have been created. Please choose the correct form based upon your association with the City from the bar at the right of the page.
The Form 700 consists of the Cover Page and Schedules A through E. Please review the list of Designated Employees, and complete the packet according to your corresponding Disclosure Category, which specifies the types of personal economic interests that must be disclosed.
Much of the information on the Cover Page (page 5) has already been filled in. Please ensure that each of the following sections contains the required information as listed below, and complete each section as appropriate.
Top Section: Name
Fill in your name, daytime phone number and mailing address
Section 1. Office, Agency, or Court
Fill in your position title as it appears on the Designated Employees List – not “working titles”
Section 4. Schedule Summary
Record the total number of pages
Check all applicable schedules and complete any required schedules OR check no reportable interests on any schedule
Section 5. Verification
Date
Sign
PLEASE BE SURE TO PRINT, SIGN & DATE THE FORM, returning only the Cover Page and applicable schedules, not the entire packet, to the City Clerk’s Office by the deadline.
The legal deadline established by the Fair Political Practices Commission for filing your annual statement is April 1, 2009 at 5:00 p.m. However, you are encouraged to complete your form and file it with the City Clerk’s Office at your earliest possible convenience.
